A tattooed arm is designed to look like a realistic exposed skeleton, with bones extending from the wrist to the fingers. The joke is that the artist interprets the client's request extremely literally and enthusiastically, creating an exaggerated skeletal tattoo. It is sent as a reaction to someone taking instructions too literally or going far beyond what was asked.
