What it means

Two simple cartoon cats appear in a four-panel-style meme. One cat tells another that it has been diagnosed with schizophrenia, after which the concerned cat asks whether it is real and realizes the situation is getting worse. The joke comes from the second cat seemingly witnessing the diagnosis become real through the strange cat figures and escalating reaction.
