A doctor is shouting at a calm man whose skull is open and brain is exposed. The joke is that the doctor recognizes the absurdly obvious emergency, while the injured person casually walks into the hospital as if nothing is wrong. It is used as a reaction to someone ignoring an extremely serious or obvious problem.
