What it means

A plus-sized healthcare worker in blue scrubs and gloves stands beside a hospital bed, apparently sneaking or handling food. The caption jokes that they were told to cut down on snacks, making their large size the punchline; it is used as a self-deprecating reaction to being advised to eat less.
