What it means

A person shares a meal while displaying a video game scene featuring a group of very different characters, including a mafia leader, a former yakuza chairman, and a formerly homeless man. The joke is the contrast between the affectionate caption and the absurdly dangerous backgrounds of the supposed “best friends,” making it a reaction image for unlikely friend groups.
