A man shows off increasingly elaborate eye makeup in a four-panel selfie collage while claiming to the guards at a psychiatric ward that he is getting better. The joke is that his supposed improvement looks increasingly chaotic and absurd, so the statement is clearly ironic. It is used as a reaction to jokingly claim progress while actually making a situation worse.
