When some weirdo at the next table starts painting you
A man at a restaurant table turns around suspiciously while a strange person at the next table appears to be painting him. The joke is that an unexpectedly intense or creepy stranger is staring at and sketching you in public, making the situation awkward and unsettling. It is used as a reaction to feeling watched or singled out by an eccentric person.
