What it means

An anime-style girl looks confused beneath a green-text story about a trans girl dating three boys who all came out as trans girls within a month. The joke is that her romantic choices repeatedly turn out to be gender transformations, making her incredulously ask why men are like this; it is used as an absurd reaction to an unexpectedly recurring situation.
