A person in a red shirt has an enormous black cockroach-like insect crawling over their shoulder and neck. The joke exaggerates how the tiny irritation of a shirt tag touching the neck can make the brain imagine something horrifying. It is used as an absurd reaction to overthinking minor physical sensations.
