What it means

A meme compares an impressive-looking fantasy trailer scene with the bizarre result of pressing random in Skyrim’s character creator, represented by a distorted image of Todd Howard. The joke mocks how polished game trailers can differ from the strange characters players actually create, and is used as a reaction to unexpectedly ugly or ridiculous character customization results.
